Driver’s Eligibility Form Request
Fill out the information below to request a Driver’s Eligibility Form. Please note, you will receive an email notification when your student’s Drivers Eligibility Form is complete. A parent/guardian must come by the office to sign and pick up the form. Forms are valid for 30 days.
This form is not to sign-up for Driver’s Education. To sign up for a class you have to visit https://www.randolph.k12.nc.us/page/drivers-education
Please do not submit this form if you have not completed both the Driver’s Education class, as well as Behind the Wheel Driving Instruction.
